The humalog don't work any more...

The Humalog is no longer compensating for my basal being wrong, it just makes meeeeee worse, but I, know i'll see your face again!

Ditto to everyone else - if you have a look at the excercise thread, there are mentions of it raising blood sugars, particularly if you don't eat beforehand. As others have said, sounds like you need a referal to a specialist, not just GP.

Good luck on your next visit - HbA1c 12?

Sorry to hear you're having such a rough time. I just wanted to add my own observations and ask a quick question.

When you say you "take 18 in the morning" and "16 at night" of your slow insulin - can you tell us the times? I may be wrong, but I think most people who split their dose split it 50/50 and take it exactly 12 hours apart.

What I'm thinking is you've split you're slow insulin almost 50/50, but most people are awake 2/3rds of the day and sleep 1/3rd. So depending what time you take your night time dose, it could still be active the following morning when you take your morning dose.

If this were happening I'd expect you to be going low mid-morning (as you've effectively got 2 doses of slow acting working), and going high in the early evening (as the morning dose could be wearing off too early).

This is just my guess at what could be happening, but I'd echo what everyone else has said already and say an appointment with a specialist nurse is definitely the best way to go.

Sorry if this sounds a little blunt but....

1) Your basal is wrong. You are proving it daily. Until you get your basal sorted, you will not have any kind of control.
2) Eat when you get up. A common mistake among diabetics of all types and control always improves once people start having something for breakfast since it prevents the liver from dumping or stops a dawn phenomenon dump from continuing.
3) The title of this thread should really be "Humalog is no longer compensating for my basal being wrong". Changing to novorapid is only going to be an attempt to patch over the actual problem. Sort your basal out and you may not need to change bolus at all.

Hahaha I do prefer straight talking actually so no offence taken at all. I think I will increase my Basal as I agree I am using my Humalog to cover the shortfall. thankyou to everyone for their comments and advice , its all appreciated !! I will sort this out during the net few days!!
 
Just a point on the exercise and food as well then. Bearing in mind from a non-injecting T2 perspective, I find a carby breakfast about an hour before exercise works best over the day as a whole. Something like porridge is ok, although only if exercising. Since the BG will peak about an hour after eating, the glucose is there when your muscle (etc) cells need it during exercise.

Now its slightly different in T1 because of the trade off between bolus and the lowered requirement for insulin during exercise. I forget the exact biochemical mechanism but when exercising cells can accept glucose with a lower insulin requirement. So it may take some experimentation to get that balance right.

Also bear in mind that when I am not exercising, breakfast does not generally involve an awful lot of carbs for me, since my insulin resistance is greater in a morning. I know you are using carb control to try and reduce your margin for error in bolus shots, but in a case where you are exercising, having a little higher BG to be burnt off during exercise may not be a problem.

Unfortunately there is another complicating factor in the form of ketones. If you have something low carb for breakfast then it may still cause a reasonable rise in BG while producing ketones. While ketosis is not a problem normally, exercise will also normally raise your BG as well and ketoacidosis is the result of high BG and high ketones.

So a carby breakfast before exercise will reduce the chances of having a lot of ketones sloshing around, will give you some BG to burn off during exercise and switch off any ongoing dawn phenomenon liver dump.

But as with many situations involving insulin its a juggling act I'm afraid :(

Hi NiVZ , thanks for your question ,I take my nightime Levemir at 10pm and my morning anytime between 7am and 8am . Can I just add that since my levels have been so erratic I hardly ever sleep , sometimes if I can I will sleep 3/4 hours during the early hours but the last two days I have only had 3 hours sleep, none at all lastnight. I seem to have developed Insomnia , I never sleep during the day so thats not the problem. I have seen the doctor after doing this for several months and he refuses to give me any medication as I am a Diabetic and might not wake if I have a nightime Hypo . Grrr :D

2. harass the doctor for a referral to the hosp

3. sort this bloody Basal out grrr

4. act on my HbA1c ,

5. that it ?? I already do the healthy eating and exersise part.

6 Tryyyyy to get some sleep !! hehe
 
haha good girl. ok i'm happy now 😉 I think that's about it. Maybe the sleep should be higher up the list though :D
