Have you tried different insulins? I couldn’t avoid spikes or hypos on any of the supposedly short acting insulins. Apidra suits me as it has a faster action and a shorter “tail”.
I still spike sometimes, but less since I realised the impact of 15-20 minutes exercise after meals. I’m moderate carb and I also find low carb meals sometimes spike me more than I would expect, especially in the evening for some reason.
One thing I do is split the carbs in a meal so if I’m having a dessert I will leave it an hour or so for example.
It sounds like you’re working really hard to get this right, and sometimes it can help to stop worrying about it for a while, after all your levels are only high for a short time and they aren’t astronomical. Good luck and keep us updated on how you get on. I would definitely ask about Apidra though if you haven’t tried it.

I have mentioned different insulin but its a no go ☹ My DSN is a nice lady but her answer for everything is 'take more insulin'

I think I need to try not to overthink things and take one day at a time. I will start writing my food diary again and try to record what I eat.

I can eat a chocolate muffin ( with insulin ) and spike less than if i eat a low carb biscuit ( also with insulin )?? Its almost a joke between my partner and i that my body seems to respond better to junk food than cutting the carbs

The junk food aside, could you not try and increase the carb intake at each meal and see if this settles things down, not talking about a drastic increase here just 20 30g more.

I know that Novorapid takes 15/20 mins to work but I cant pre bolus for the same time for all meals. If I am in range and I pre bolused 30min before eating I would hypo before eating, if I was too high 30mins would not be long enough!

I know my correction dosage and I have different ratios for morning, lunchtime and evening meals.

I also do a split bolus if needed
I find that I time my bolus depending on my BG. I test 30 min before the meal and then time my bolus according to whether I am above, on or below target, reducing the amount of time for lower BG.
Would that help at all?

Hiya. Yeah I test about 30min before I eat so I can bolus accordingly. If in range I wait to bolus untill 10 min before eating, if too high I inject and wait 30 - 40min before eating.

If the timing you are using still leads to spikes then you could try altering them.
It took me time to get mine sorted as it is different for each of us.
If I am
- above target I will deliver bolus (carbs and correction) and get back in target before eating
- on target I usually deliver about 30 min before the meal,
- below target I will wait and deliver just before the meal.
This is all adjusted according to the type of carbs that I am going to be eating.
