Have you tried different insulins? I couldn’t avoid spikes or hypos on any of the supposedly short acting insulins. Apidra suits me as it has a faster action and a shorter “tail”.
I still spike sometimes, but less since I realised the impact of 15-20 minutes exercise after meals. I’m moderate carb and I also find low carb meals sometimes spike me more than I would expect, especially in the evening for some reason.
One thing I do is split the carbs in a meal so if I’m having a dessert I will leave it an hour or so for example.
It sounds like you’re working really hard to get this right, and sometimes it can help to stop worrying about it for a while, after all your levels are only high for a short time and they aren’t astronomical.
