What might motivate teenage diabetics?

I'm 16, been diabetic for 9 odd years now.
And i've just moved countys and had my first diabetic appointment here yesterday and got told that i should keep testing cause i dont.
and to eat right.
tbh, i need some sort of guidance but i'm not going to get it from home as my mum has my sister who is 4 and my brother who is 3weeks old to look after so i'm on my own with this, i've sorta just let my diabetes slide as i've had so much else to do, with my gcses, moving and helping my mum out.
So i really think there should be some sort of help, for people who like in my situation has forgotten how important diabetes is. I mean i got told if i don't change how i'm dealing with my diabetes i will become blind or lose a limb within the next 3-4years.
