Hi,

Once I had established my daily dose after a few weeks of starting tresiba, I haven't had to make any changes since then. I've been on 19u daily for past few years. I only adjust my bolus (mealtime) if and when I need to, generally if i've done some vigorous excercise I'd reduce my bolus by 50% for about a day and a half. To be frank, I wouldn't of gone on to tresiba if I felt I would need to change it regularly. I'd probably prefer to go a on a twice day basal or at least a basal that doesn't take 2 days to take effect.

I was advised to wait a week between dose changes when i took it. The dose lasts up to 3 days so it takes 3 days until a dose change takes effect, and then you need a few days to do more basal tests and collect data before having enough evidence to decide whether it needs more changes.

I would do a base test, change the insulin by the smallest possible amount if needed, and test again after four or five days on the new dose.

It is better to make the change if needed in a number of steps rather than attempting to do it in one large step.

Yeah tresiba stays in the system for around 3 days so it’s best not to be tinkering too much. It’s not ideal to be changing the basal lots unless there are obvious long term changes in basal requirements. If you’re finding that your basal needs change a lot you may do better in a basal insulin that doesn’t have as long in the system. Tresiba is especially difficult to manage if you do exercise and things like that.

Same here @Lucyr I was told same thing. After changing a dose we need to give a few more days to see what effect it is having. Thus, a week reasonable.
