Hi all,
I have been told by my DSN i needn't bolus for breakfast. But I have recently noticed that 2hrs after breakfast my bg is between 10-12. By lunch I am normally back down between 5-6. It dosnt seem to matter what I have to eat.

Do you think I should bolus for breakfast or will I go to low by lunch?

My DSN says it's only a problem if it dosnt come down.?
Any thoughts or advice much appreciated.

Original I was told my Lantus would last all day and only bolus with evening meal.
After 2 months of almost having nothing for lunch to keep my bg low, I asked if I should bolus with lunch. They said yes and now I think I need to with breakfast.

I am not really type 1, probably type 3 due to necrtosing pancreatitis and maybe they weren't sure if I was producing any insulin myself.?
 
Hi Mike, a lot of us, whatever type, have problems with a spike after breakfast, because the liver tends to dump glucose into the system to prepare for the coming day. Sorry, that's not much help to you! The danger is, that you might go low by lunchtime, if your own body is capable of bringing you back down, albeit slowly. What I'd do, is experiment with a very small dose with breakfast, increasing it if necessary, and make sure you test mid morning and eat some fast acting carb if you're dropping.
 
Hi Robin, thanks for the reply, only noticed that I was spiking after breakfast this weekend.

I normally have breakfast at 06:00 on work days then have a cereal snack at 09:00 in work then by 12:00 I am between 5-6. So i though i was doing ok. But this weekend had breakfast at 08:00 and took bg at 10:00 my readings were above 10 but dropped back down by lunch.?

My readings before breakfast have always been sub 6.

Hi Mike, I'm also type 3c, due to distal pancreatectomy. It's difficult as we probably are still producing insulin but in fits and starts. I bolus for every meal if it's over 10 grams of carbs. It's trial and error I'm afraid. It sounds like your DSN doesn't understand your type of diabetes which isn't surprising really. My DSN is petrified of me having hypos and likes my HbA1c to be a bit higher than the diabetic cutoff ie 48. Have you been diagnosed long? I'm 7 years in and my remaining bit of pancreas is atrophied and I have noticed in recent months I am having to inject more units than previously. I would talk to your DSN again and ask for some more clarification. Hope you get sorted. Elaine.
 
Hi eggy,
I was admitted to hospital April 2016 and spent almost 4 months there 8 weeks of which were in intensive care.

My last hblac was 41 and that's when the DNS told me to aim closer to 10's with my bg?, I was have only seen the nurse twice in 12 months and am not really sure what I am meant to be aiming for. I feel the effects of a hypo when I get into 3's and have only been sub 3 once .

I will talk with my DAN again, thanks for the replies.
 
Hi there @Mikew. I'm a type uncertain 1/3C myself and my insulin requirements have varied a lot since diagnosis almost 3 years ago and at one stage I could do breakfast without bolus and without spiking. The key factor was what the breakfast was. Slower release carbs seemed to work best for me.

What is your typical breakfast?

Your HbA1c is very good and I suspect that's why your DSN didn't want you bolusing with your breakfast. It's almost 10 years since my op and I've been on insulin less than 3 years and only on MDI for less than 1 year. My little bit of pancreas was obviously still producing enough insulin, even though I lost the tail which is where the majority of insulin producing cells are. Because it's all but conked out now my insulin requirements are much higher and to my disappointment my HbA1c was 52 at my last review up from 49. My DSN is happy at that. I'm not! I tend to avoid cereal type breakfasts as they spike me terribly and make me quite unwell. Have you considered a Freestyle Libre? It may put your mind at rest about the spikes, it did me. If I ate porridge, for example, even when bolused for I could spike up from say 6 to 14 in a very short space of time, that's when I understood why I felt so ill! But it also showed it did drop quite quickly. Without the Libre, I may have changed my breakfast ratio and subsequently could have suffered a hypo before lunch. There's loads of threads about the Libre. There is a cost unfortunately but it might be worth a try. I have heard of people been loaned one. In the case of us "weird" diabetics, and I use that in an affection way, I think we need all the help we can muster. Take care. Elaine.
 
I'd second the Libre even if you can't afford to run one full time. The insight they give you on your BG patterns is very useful.

I have been looking at the libre , a work college has one.
My breakfast mainly consists of either 1 piece of burgen bread toast at about 06:00, then lizzy's low sugar granol about 09:00 . My bg's by lunch were normally between 5-6 at midday. But it all changed over the last couple of weeks and my levels have increased.

Once the apidra is on board I am able to keep to good numbers.

I have had a bowl of blubery wheat this morning with 2 units of apidra and my bg was 5.9 2hrs later. So will try this again.
