Hi,

Recently found out I’m pregnant (roughly 6 weeks).
Bloods were perfect pre pregnancy but now I seem to be sitting in the high teens post meals and over night this is my first pregnancy and I’m super anxious and also super frustrated given my levels were so well controlled pre pregnancy.
I’m using a Medtronic insulin pump with guardian 4 sensor with a closed loop system.
I have spoken with the diabetic team and adjusted carb ratios but changes don’t seem to be making a difference.
At present I’m over riding the pump to give extra correction doses/boluses and today I even had to use an insulin pen to deliver to a correction dose as pump corrections were taking too long.
Luckily no ketones.

Hi, congratulations!
I am 15 weeks pregnant and also using the Medtronic 780g with guardian sensors.

It might be worth reviewing your carb ratios. At this stage my carb ratio was doubled but my DSN thought it was probably inaccurate before - the Medtronic is very good at correcting which can mask inaccurate carb ratios, but these can come to light during pregnancy when requirements become a little more intense. Although if highs are also overnight this may mean your basal just needs a few days to catch up and will readjust to meet new insulin requirements. What sort of levels are you experiencing and when do they tend to settle down overnight?

Hace you been in contact with your diabetes team? Hopefully they will be supportive in helping to get your levels down. They may offer to switch you to the Ypsomed pump as this is particularly recommended in pregnancy and the algorithm is set at a lower baseline target (of 4.5 I believe).

over the last week my active insulin time has been changed as well as my carb insulin ration for lunch and dinner. my diabetic team have been great so far but unfortunately with it being the bank holiday weekend I haven’t been able to speak to anyone.
today the bloods seems more erratic rather than high. fluctuating between highs and lows which is super frustrating.
I’m hoping it’s just a blip with the bloods and il get back on track but I must admit I am very overwhelmed by it all and in turn becoming very anoxia’s and emotional which is also isn’t helping the bloods.
 
Bless you, it sounds as if you've ended up yo-yoing a bit trying to get on top of it all (which I can relate to!)
Probably best to try to stay calm, you're already doing the best you can and you're right, being stressed doesn't help sugar levels! Tuesday will be here before you know it and it sounds as if your team will be able to give you good support then.

I am using the Medtronic with the Simplera Sync, but not pregnant.

You mention that you are overriding the pump and also using your pens when hyper. I have found that the system takes a bit of time to catch up when I am ill and have found that the corrections are slower when I am high, which it always was when before looping. When you are adding in extra insulin are you doing this whilst in manual mode or still in Smartguard? The loop depends on knowing what is going on and adjusts its decisions based on the info from the previous three days so it takes a bit of time to settle into the pattern you need. Well worth chatting to your team about how to manage the highs.

You also mention that your team have changed the ratios for you. Have they also taught you how to review and make changes yourself? Again worth having a discussion about this.

I found that changing my action time for the insulin helped to address changes that I needed as the system wasn’t doing what I wanted it to. The action time for the algorithm is not the same as the profile for the insulin that you may well have seen for whichever insulin you are using. The shorter the action time the more aggressive the corrections are, as the algorithm thinks the insulin you have on board has run out more quickly.
