In this randomized clinical trial of 405 adults, the 5:2 MR approach achieved better glycemic control at 16 weeks compared with metformin and empagliflozin.

An intermittent fasting plan consisting of 2 nonconsecutive fasting days and 5 days of habitual intake per week and meal replacement diet (5:2 MR) could provide additional benefits to patients with type 2 diabetes.

5:2 MR (Meal Replacement) used shakes, like those in the Path to Remission programme. Shakes have to be used in medical studies for consistency. They are appropriate for professionally supervised dieting, and not advised for self managed dieting.

Does this suggest a standard 5:2 weight loss approach is a good alternative to medication for prediabetics and newly diagnosed T2Ds?

The this graph and the newspaper summaries don't mention the effects of different doses of metformin compared to the 5:2 diet.

Administration of Metformin or Empagliflozin
Patients took metformin (Shanghai Bristol-Myers Squibb), 0.5 g, twice a day. If the initial drug dosage was well tolerated, it was escalated to 2 g per day. Empagliflozin (Shanghai Boehringer Ingelheim), 10 mg, was administered once a day. During the study, patients were instructed to promptly contact the research center’s physician in case of severe hypoglycemia.

Interestingly the original DiRECT study had an improvement in HbA1c of 0.9 in the intervention group (7.7 down to 6.8) which was poorer than all three of these interventions. With 5:2 (1.9) and Metformin (1.7) about twice as good as Taylor's crash diet.
