Hi,

One of my problems is getting my pre breakfast blood sugar to below 10 mmol/L prior to breakfast. In the evenings prior to bed I test my blood sugar and give a corrective dose according to what my readings are. I work on the principle that one unit will reduce my blood sugar by 2.5 mmol/L. Sometimes this works but most of the time it doesn't. I've just upped my night time Levemir by two units to thirty four. Does anyone have any suggestions?

Hi Tom!

I know we are new to all this but- Alex had the same problem and so his DN told us to increase his night time levermere from 8 to 8.5 - it has taken about 4/5 days to get his levels down - but it seems to be doing the job as his levels today were between 5 and 9.9 which is a million times better than last week!
But we have been told that 1 unit equates to 3 mmols not 2.5 so maybe that is having an effect? Bev

Tom,
Before you go increasing your levemir do a check at 2 or 3 AM just to make sure you are not going hypo and then having a rebound.
Do you split your levemir? If not worth thinking about or even changing the timing of it. What ever changes you make leave it for 3 days before you change anything else.

My Levemir is a split dose job. Thanks for the tip to check at about 2/3 am. I'll leave my night time Levemir for a few days until I do anything.

Tom i am no expert and still working on getting my levels better too. I changed to basal bolus about 2 months ago now. Really struggled with morning readings and so frustrated so, kept upping my basal. However, found i was going low occasionally before lunch and tea? have now been decreasing my basal back down as it did nothing to bring down pre breakfast readings, however have been correcting before bed and injecting for supper, so usually 5 injections a day most days. I have been using a correction of 1 unit for 3mmol and fingers crossed have had near perfect breakfast readings, for about a week, would look at this before upping levemir. the way i see it the basal should keep things steady between meals and not lower you too much, use the fast acting for this. hope this helps.🙂
 
high morning readings

I have also struggled for years to get bg below 10 consistently in the mornings. At one time I kept putting up the Lantus then decided possibly going low through night, difficult to prove even by testing at 3am as readings vary a lot. Over the last few months I've reduced Lantus from 24 to 17 and have a lot more readings below 10 with occasional blips probably depending on the previous days activity levels. Just have to keep trying. Good luck

I would be checking at 2 am ish if I were you as well. Test before you go to sleep and then in the early hours. You may have to do this for a week or so to see if you get a pattern.

I would say you were a prime candidate for a go at a CGMS. You will be able to see what is happening overnight for hopefully a 5 to 6 day period (once the data has been downloaded). Ask your hospital, they should have one or two machines and everyone is entitled to have a go.

If the Levemir still isn't working for you, ask to change to Lantus and try that. I have lots of friends whose children have had to swap from either Levemir to Lantus or the other way around and different ones suit different children.

I think Tom has had a CGMS before. Can you remember what it showed for your overnight period? Just wondering if you were experiencing high waking blood sugars at that time?
