Lantus - split dose back to once a day

Back in February I was asking about splitting my Lantus from a once daily dose to am & pm. Looking back through my BG diary I started with the split on Thu 21/02 - Fri 22/02.

I'm now going back to taking Lantus/BI once a day - it's designed to be a once a day insulin & not twice/split like Levemir.

This came about after going to my diabetic drop in centre yesterday & chatting to the DSN who spotted lots of "little" hypos all over the place - I'm taking my bedtime dose then the morning dose then also QA(meal times) inbetween times so the insulins may at some point be overlapping & causing the hypos.

So as directed by DSN:
- 10am yesterday morning, already had - 10u Lantus
- 10pm yesterday evening, take - 18u lantus
- none this morning
- 18u this evening
- continue with once a day dose at 10pm
- if hypos are still happening reduce lantus to 16u
 
Well, lantus is supposed to be once a day, but in practice many people fnd that it does not last the full 24 hours. I'm surprised you're changing it back Gill, as you have been having much better levels since you split it.
 
I think you've got to find whatever system works for you. But then you can't just assume that the doses of that system will work perfectly.

In your shoes if I'd got better levels on split Lantus and was now getting lots of little hypos I'd be adjusting the doses of the split(s) rather than going back to an old system that had not worked for me in the past.

All you can do is to try it though!

If you end up with problems going back on one dose you can always revert to a split and adjust from there 🙂

Possibly in some people it may only last 22 ish hours - think it does with me 😱
Yes I've had much better levels (fasting/waking & bedtime) apart from all the nasty little hypos 😡
Anyway I'll give it a go and see what happens 😱

Adjusting the doses of the splt is/was an option.
Like you say I can always go back to the split doses.

In a few weeks it'll probably be "help this once daily dose isn't working for me"
