It is good that you have an appointment with your DSN coming up and you can start to gather questions to ask. With the fixed doses you are on, you will be needing to match the amount of carbs to the dose they have given you. If you are going low after breakfast or lunch, which you mention is generally the same each day, then you either need more carbs, or less insulin as these are not balanced at the moment.
It will become a lot easier once you are able to match you meal time doses to what you choose to eat. This will require you to know how many carbs are in each meal. This seems like a lot to sort out the start but you will be surprised to find out how much of an expert you become on this. So starting on that journey is a good thing to do now. Perhaps start with breakfast and lunch, and get those sorted and adjusted.
Dec 9 seems a long way off and as you are already seeing patterns in your levels, could you phone them or email your consultant to get some advice?
