Are you seen at a hospital diabetes clinic or by your gp? If it's not at a hospital ask for a referral to a clinic where there are specialist consultants and nurses to help you get to a better place. If you do have a diabetes team get in contact and ask for help in getting control back for a healthy, happy future.
Diabetes is tough and from my experience I found growing up through my 20's really difficult with everything else going on in my life. You obviously want to improve things and need some support and help to get you on the right track and on here we all understand the challenges and have been through difficult times.

When you say sometimes you haven't injected for weeks, have you been hospitalised with DKA during that time or are you taking your Levemir ? As an insulin dependent diabetic you can't go on for long without any insulin and without being desperately ill. Are you not injecting for any specific reason or is it you just don't know what to inject for the food you're eating? Whatever the reason get in contact asap with your care team and tell them how you are feeling and what you are doing. They will have seen it before and will want to help you get to a better place and help you with carb counting, injections, correction doses, testing etc. Once you get to a better place diabetes doesn't seem such a frightening muddle and you will feel so much better with stable glucose levels. None of us know it all however long we've been dealing with diabetes and often it's just assumed that you know what to do for the best because you are somehow managing to get by. There is absolutely no shame in saying I've lost my way and need help with it all .
If it all seems like a great big overwhelming mess, break it down into bite size pieces - testing, when you wake up, before/after meals, before bed and record the values even if they're not very pretty. Have you ever been shown how to carb count? If not, have a read here for the basics and urgently request help from your team to be shown how to match your insulin dose to your food choices .

Please don't despair and get overwhelmed by it all but please do contact your Diabetes Specialist Nurse if you have one or your gp for a referral. The amount of effort it takes to get things sorted now will pay off massively for your happy future.
