Hi all,
Been type 1 for 23yrs now, no issues lately but was wondering if anyone else on the non analog insulins had trouble after completing a dafne? I gained 2st in 6weeks and shot my Hba1c from 70 - 84. I quit the dafne guidelines and managed to get it back to normal but still having trouble losing the weight gained.
 
Sounds like you aren't counting carbs and adjusting your doses, as you quote fixed numbers for meals. Did your HbA1c get worse after DAFNE? Sounds also like you have a touch of insulin resistance, those are quite large doses. Can I ask how long it is since you were seen by a specialist? Those insulins are a touch prehistoric, by which I mean they were around when I was a GP.

I was previously on novorapid and glargine and doses I was on were huge, averaged 150u of novorapid a day and 60u of glargine, lost all hypo awareness and they caused me to suffer seizures. I was then able to get to see a specialist at UHND and he changed me back to Insulatard and Actrapid as the Actrapid lasts longer in the system, my dosages fell dramatically and I gained a much better hba1c from 90 to an average of 65. I gained hypo awareness again and the seizures ceased within 8wks.I also lost a little weight going from 18st to 14st 4lbs in 3 months. They then suggested a dafne as it may help me a little more. The nurses that ran the dafne admitted it was not tailored for the insulins I was on and that I may have a few issues to start with but after the 6wks I went back to see the specialist and he recommended I didn't follow the dafne guidelines which has helped my hba1c and weight a little but still finding it hard to lose the last 1st of weight I put on. It was the specialist that I discussed changing from actrapid to humilin s as the actrapid was vial and syringe only which was a pain when out and about and the humilin s has the same profile as Actrapid.
Last seen a specialist in Sept 2016 and due back in Sept 2017
 
You need more insulin for dinner than I use in a whole day (TDD, basal and bolus combined) Despite the fact that I'm obviously smaller and lighter than you - I think the same as Mike - sounds like there's some insulin resistance going on to me too.

Where this happens - Type 1s often get benefit from adding Metformin to their diabetes repertoire. One T1 lady I know is carrying some excess weight - so hiking up her insulin doses just exacerbates that - and a T1 chap I know is on various mental health drugs which hike his BG up but can't be avoided - it's a long standing condition and he and 'they' are always trying a different cocktail and he always has to come back to the ones that cause the BG prob in the finish so he's had to accept it if he wants to function - and metformin tablets help both of them quite a lot. This applies equally to any number of folk.

Had adding it ever been tried or suggested ?

My insulin intake has actually dropped quite a lot since going back on the old style insulins. Been tested for insulin resistance and they say I do not have it. Metformin is a definite no go as it does not play well with the Humalog based insulins I am on, they tried it when I was on the analog insulins and it had no effect so was quickly ruled out. The issue I have is losing the weight, I was fine where I was at before DAFNE, yet once the carb counting started it all went to pot, higher hba1c's and increased weight along with larger doses of insulin. But as I have already stated the dafne course is for the shorter/faster acting insulins. My meal time insulins currently stay in the system for up to 8hrs where with novorapid,etc they are used up after 5hrs. I have cut my carb intake quite significantly, but am still on a ratio of 1 carb portion to 6units of insulin at meals. I even have to inject if I have a carb free meal though this is usually a small dose of around 7u for a 3 egg omelette.
 
Hi I use animal insulin in a pump and count carbs just the same as everyone else does. In fact I have counted carbs or my mother did from 1965 which is basically what DAFNE teaches you so no idea what the problem is.
