Following on from my troubled times thread 2 3 weeks ago I've recieved an appointment with a mental health team for the 3rd July and even better it's at my gp surgery just in a smaller office type thing adjacent to it. Happy to say my GP continues to be great

With the questions you need to answer, it might be helpful to write some notes for each question, then reflect for a day or so before you fill the form in.
