GEN1 Form - No consultant!

Hello there, shame my first post isn't an introduction but I'm in a mild panic. I'm currently filling in the forms for licence renewal as my 3 years are up. However since I've moved to my current location I haven't been getting seen by a consultant at all, all of my diabetes management has been through the practice nurse who has been nothing short of wonderful.

However now I've gotten to the last section of the GEN1 form, part B, it's asking for the name of my GP and the name and address of my consultant. Now I've already retrieved the name of the GP who specialises in diabetes at my practice from the practice nurse and I've popped him into the form. But I've never actually been seen by him, so I don't know what to do about the date. As for the consultant section I just don't know what to do, I'm terrified of leaving it blank and somehow losing my license as there seem to be a lot of horror stories going round currently on diabetics getting pretty thoroughly bashed by the DVLA. :confused:

If anyone can offer me any advice at all it would be much appreciated!
 
Hi Grulgor, welcome to the forum 🙂 I don't drive myself, but I know that not everyone has a consultant - many people do as you have been doing and see their GP and/or nurse, so it can't be compulsory to include a consultant's name.

Hopefully someone will be along to confirm 🙂
 
I'd put the names of your nominated GP (we all have one LOL) and the nurse, meself.

There's quite a lot of diabetics 'out there' who don't have consultants and are in the care of GPs anyway. And there's no guarantee they will even contact your doctor in any case.

In my case, apparently my consultant didn't send the form back after we discussed it, and the first I knew of that was a phone call from my GP (not an office minion, actually my doc) to say he'd had a form from DVLA to complete and would therefore need to see me so he could fill it in (the same form my consultant had in front of him months previously) I then contacted the hosp who confirmed there had been a lash-up and arranged a pdq appt to update the form. Then rang the DVLA, explained what had happened and did they want both or just the one from whoever and they said they didn't mind who it was as long as it was up to date, factual and signed by someone who could add the details of their GMC registration !

So just be truthful and delete the word 'consultant'.

Don't you have form DIAB 1 ? Do you have other stuff as well as D ?
 
I'd be honest as trophywench says - put the name of your GP (but mention on the form the nme of the diabetic nurse that actually sees you) and under 'consultant' part simply note that your GP manages your diabetes.

They ask for both solely in cases where a diabetic is managed by both a GP and a clinic (as I am) and its just to make sure they have retrieved all information about you. I would imagine that in your case, the would simply contact your GP to confirm that they are the sole carers for your diabetes.

I guess if I did put the nurse in I'd have to cross out the automatic 'dr' title they stick in there too, still half wondering if I should just leave it blank though.

I've filled in the other forms DIAB1 and D42 with no hiccough's at all, as DIAB1 is much more sensible and lets you tick practice nurse. I think I'll have to make a mock appointment with the GP just so I have a date to put into the form, plus I can sneak in asking him about it at the same time then just incase he's had this happen before.
 
on a related note, since I registered in Surrey (been here 6 years now), my first diabetic GP left after 2 years of me registering with the surgery, the second appointed one has just left this year, and over in the hospital, the head of the diabetic clinic left last year.

was it something i said??? 😉
 
If, as I said Dory, they actually bother contacting anybody about you anyway.
 
I don't have a consultant or a DSN, practice nurse needs to do just that practice, but not on me. I just put my GP's name on the form. He has never been contacted.
 
LOL @ Sue, my diabetes MOT at the docs on Monday and they have a new nurse ! should be fun, actually, esp if she's as much of a complete idiot as the other one.

(first meeting in 1998 - How long have you been diabetic? Well you of all people should know you can never, never, NEVER ever eat sugar! And this was because I regularly had the tip of a teaspoon's-worth of sugar on my morning Weetabix, which is 18g carb with the milk, so I used to think I'll just nudge it up to nearer 20 with those 10 grains of sugar cos it's easier to bolus for, innit?)

My word I know my first nurse / doctor was pretty bad but that just takes the biscuit. Current nurse is fantastic, but we're looking at moving again soon'ish so fingers crossed I don't end up with one of those nightmares.
 
Why don't you make a 'real' appointment with your GP and ask him what you should do.
