Following sick day rules but...

I am ridiculously poorly. I thought I had a cold but today I can't swallow, my neck is super swollen and I keep going dizzy so I'm thinking maybe it's tonsillitis.

I coughed so much I threw up earlier but I haven't eaten anything so it was just bile. Needless to say my throat is VERY SORE.

I still took my Levemir basal this morning. I ate some ice cream to soothe my throat and inject a tiny bit of insulin for it. That was this morning. I'm now sat at 13mmol with ++ ketones. If I ring 111 I know they'll just send an ambulance when I'm thinking my ketones may be starvation ketones due to lack of food? If I can't stomach food, do I still do a correction Dose? I'm concerned about hypoing and struggling to eat food.

Any advice appreciated 🙂
 
If I was sitting at 13 I would do a correction. Perhaps consider doing half your correction if you are worried. Being so high you would have insulin resistance as well for an added bonus.
Can you suck a small piece of ice to help with your sore throat? Or perhaps gargle some dissolvable aspirin if you can use it.

Hi Rosie. I am going to attach the DAFNE sick day rules flowchart. Hopefully you can make sense of it. Basically, contrary to all instincts you need to carry on doing fast acting insulin - in the flow diagram this is what QA stands for (quick acting). You just about fall I. The severe illness side of the flow chart. You need to calculate your Total Daily Dose from yesterday (basal and bolus) and give 10percent of it. I have to say that I have not got sick since doing DAFNE and can totally relate to being scared to hypo and not keep anything down. At least do try a correction dose. Keep us updated and get well soon .

Thank you everyone. I must have looked so pathetic in the chemist cos the pharmacist went "do you want the strong stuff?" ha ha yes please!

Thanks @Pumper_Sue I took your advice and did half my usual correction dose. Will wait an hour before testing again to see if it did anything! I take my nighttime Levemir at 9:30 so I only took half of that too. Think I'll be setting a 2/3am alarm to check I don't hypo since I've eaten nothing since ice cream at 11am.

Thank you for this! I will definitely be saving this picture to my phone for future use. It does concern me taking fast acting insulin when you don't eat!

If your Levemir was properly titrated before you were poorly Rosie and you weren't plummeting during the night - then I'm not at all sure only taking half of it will help whatsoever. It's likely to do the opposite!

The Big Note across the bottom of the sick day rules states very firmly 'never stop taking your basal insulin'.

The feeling sick thing - is the ketones kicking in - nausea and dizziness are sure signs of it. 'Starvation' ketones do NOT do that to you.

Rosie...can't offer you any advice on the insulin...see several members have done that already...just concerned...hoping you managed to balance those levels...wondering how you are this morning...keep us updated...if you're no better this morning...why not call 111...just to be on the safe side?...take care.
 
As Trophywench highlighted, continuing with basal insulin is vital, even when ill / not eating.
Sucking ice cubes or gargling with hot salty water both help soothe a sore throat. Not at same time, obviously!

Hi Rosie,
this is a bit late now but never reduce your basal when sick unless lots of hypos as all you will do is cause DKA.
Your concerns about your bolus insulin I addressed by suggesting you reduce the correction amount only to alleviate your hypo fears, your bolus insulin is for corrections as well so no chance of a hypo with your high numbers.
Hope you are feeling better this morning and checked your bloods throughout the night and acted accordingly to your numbers.
