Hi guys,

Two quick questions, just in case anyone knows, before I get hold of the DVLA helpline tomorrow (at some point......)

When filling in the dreaded DIAB1 form it asks for date last seen by GP - does this include the DSN at GPs or physically my own GP?

Also in the eyesight section Q4 reads:
Have you had laser treatment or injections into both eyes.... Does this mean 'laser treatment' in either eye or 'laser treatment or injections' in both?

Insurance co says I need to get letter from GP as well and he is on holiday for a week, so I guess I'll need to wait before starting the new regime

All good fun this starting on insulin business isnt it... .

Hi Malc, have a look at the top of the forum there's a handy guide for you to read.
Give the name of your GP, the surgery address and phone number. If you also see a consultant for your diabetes care, you need to give us their name, the hospital name, address and phone number. ● If you see more than one consultant for your diabetes care please give their name and address on a separate sheet. ● Tell us the date you last saw your GP or your consultant about your diabetes. If you do not see your GP about your diabetes this question does not apply so please answer ‘N/A’. ● You may give the email address for your GP and consultant if you have it, but it is not essential for our enquiries. ● Give your hospital number if you know what it is, it will be helpful if we need to write to your consultant.

As too the eye question if 1 eye has had laser treatment then you don't need to tell them. If 2 then you do if 3 then you will be a prize exhibit :D
 
Thanks Sue, have read a few guides now but just uncertain what to say for the Doc question as havent seen my GP for about 3yrs as my practice has excellent diabetic clinic team and I am seen by the DSN instead.
I am presuming I can give them that date but thought worth an ask.
Cheers for reply and thought provoking comment about the third eye.....
 
In case anyone else was wondering - DVLA advised that by last seen by GP, it didnt need to be my GP, just when last by the practice about diabetes.
