1-7 regularly.

8. Never smoked.

9-13. Not applicable or not in this area.

14. If needed I can see a DSN. And did used to attend specialist clinics.

15. Never been asked if I needed it but it would be nice to have access to. I was accused of being obsessive about testing but wasn't offered any advice or support as a result. 🙄

So, yes on the basics but no on the 'extras'.

All except 8 ,11 & 12 n/a to me

I can say yes to practically all of them apart from no 15 oh and number 12 i couldnt comment on

I'm unclear about the care plan which apparently should be discussed and agree. That's novel. My Dr tells me what it should be. I've never had one who asked me if I agreed with that (be it TSH, A1c or Total Chol)

I have definitely not been offered specialist psychological support, especially when I needed it. I eventually got some counselling but as half of my prob was diabetes related and the other half work and family, I only got help with 25% of my prob, ie work stress, since she couldn't change my family (the members who cause the problems) Or rather my 2nd husband's family.

None of this made me ill on it's own .....

So I had the nervous breakdown, joined a forum who taught me stuff about diabetes I hadn't got a clue about, packed in work and recovered - and years later I still have trouble with the rellies ......

I've filled out the survey too 🙂 I hope they get some valuable feedback - this is our time for our voices to be heard!

Ok, here's my answers:

1,Get your blood glucose levels measured - yes
2,Have your blood pressure measured - occasionally
3,Have your blood fats (cholesterol) measured - only for annual check up
4,Have your eyes looked at - yes
5,Have your legs and feet checked - never had my legs checked in my life!! feet haven't been checked in about 5 years
6,Have your kidney functions monitored - not often, but that is my fault as I end up forgetting to do a sample
7,Have your weight checked - yes, every time they tell me I'm overweight - great for someone with an ED and only a size 10/12!
8,Get support if you are a smoker - only if you see a lecture as support!
9,Receive care planning to meet your individual needs- no. up until last year, my 'care' consisted of my annual 10 minute review at the diabetes centre once every 15 months
10,Attend an education course - no. I didn't even know they did them until this forum. I'm now booked to go on one, but only because I asked about it.
11,Receive paediatric care if you are a child or young person - n/a
12,Receive high quality care if admitted to hospital - n/a
13,Get information and specialist care if you are planning to have a baby - n/a
14,See specialist diabetes healthcare professionals - yes, now I do...DSN and dietician
15,Get emotional and psychological support - no

As newly diagnosed I had all the blood tests, eye test(van came to surgery), foot tests, weight, waiting for a vist to Podiatrist but think nurse has forgotten to book, and was asked how I felt about having type 2 (that must be the psychiatric bit)🙂

I see my specialist DN at the surgery, twice in 2 months and next visit 3 months.

Having had ME fo over 20 years, and received no support beyond the initial consultant tests, I am impressed with the care and attention I have received for my diabetes so far.

Not had the offer of a course, and restricted heavily on strips, but no real complaints.
