Sorry to hear this Ingressus. What kind of levels are you getting presently? Not sure I understand ‘need 4 injections see you in 5 weeks East’.

Hi Amigo hope your well, i was supposed to have a review last month but they canceled it, oh and forgot to tell me so i turned up ok i get it happens, i am on humalog x25 twice a day which is just not working carnt remember the last time i was in single figures so i have been ill for longer than i can remember sugar high and you know how that feels, rang today for help the db nurse with bad attitude just says oh you need another type of regime 4 injections, see you in 5 weeks in the mean time i will have to suffer

Hi Simon. Good to have you back again 🙂 Is that 25 units of Humalog? and they have now asked you to take that 4x per day? At what times of the day?
Sorry but I can't remember whether you carb count for you food?

That’s shocking Ingressus. I think you need to make your serious concerns known. Maybe you’re actually being too reasonable with them. Can’t your doctor help?

Sorry to hear about the delay. Even if the nurse was less than sympathetic, it's quite possible they are as frustrated / disappointed as you. Starting MDI / basal bolus regime takes a fair amount of tweaking to get right, including probably daily phonecalls between you and DSN. If the team is depleted because staff members are on leave for holiday / study / new parenting or off sick, it may be better to wait until more help is available. Did you ask about getting prescription of new insulins and pens from your GP?

Hi Greyhound Girl nice to see you again i take humalog x25 twice a day 20 on a morning and 28 on a night 25% fast acting, they say i will take 4 injections a day under another regime Boul something lol i dont carb count one i dont know how, and secondly dont need to on fixed insulin maybe i should must admit worried about four, i have a bit of an erratic lifestyle

I understand Ingressus but sometimes we do have to assert our needs and this is the time! If you can’t get help, see your GP or walk into A&E if you’re feeling really unwell. Best wishes.

MDI is made for an erratic lifestyle! It is much more flexible and you just need to inject to cover the amount of carbs you eat. The DSN should go through carb counting with you and you may be offered a course on it. You can miss a meal if you want and pretty much eat when you want.
It is scary going from something you are not used to and don't like doing anyway, to having to do it more often, but I promise you it is a much easier regime and more flexible, and makes life so much easier.
We are all here if you have any queries about it.
In the meantime, as Amigo says, your care is awful, and it may be worth tying to get your GP to push them for an earlier appt.

I understand their position its sad for all concerned i never thought of going straight to GP but im also scared of the thought of the new regime and i wouldnt know what im doing

Let us know how you get on and if you can get an earlier appt. Keep in touch 🙂

Sorry to hear you're having such a bad time @Ingressus :(
I find carb counting so much easier than fixed doses, I have an erratic lifestyle too as I work nights on a floating rota so my poor body clock is all over the place lol! Are you on a long acting insulin as well?
I agree that you should see your GP and try to get a referal to a hospital DSN and push to go on a carb counting course or get advice on carb counting.

Hi Lucy hope your well im on humalog x25 25% fast acting and 75% long acting but fixed so there is no correction

Oh I see, sorry I'm not familiar with it, so it's a mixed insulin. I'm fairly new to all this myself but I agree with @Greyhound Gal that MDI would probably suit you better. I had to try 3 different basal insulin's until I found the one to suit me best, sometimes you have to mess about trying different regimes to find the best fit for you and your lifestyle, however, I'm T1 so it may be different.
I really hope you get some help with this asap.

Yes, sounds like your DN was sensibly suggesting a change to Basal/Bolus (MDI) but she could have been more helpful with advice and an earlier appointment.

Well - she sounds about as helpful as the proverbial chocolate fireguard, doesn't she?

The MDI regime actually makes one HELL of a lot more sense to anyone with a brain, than mixed insulin and fixed doses does, Ingressus - for the simple reason all the calculations etc are totally based on fact and logic - not on the guesswork of what someone who isn't you, thinks you might need.

You know we'll try to help when you get there.

Tell me - have you ever tried just upping the morning dose from 28 to 29 and watching closely to see how that affects you?

Hi Simon. I was going to say the same as Jenny re that nurse being as helpful as a chocolate fire guard
It can seem scary changing from what you’ve been used to, to a different regime but Basal Bolus has many benefits over mixed insulin or fixed doses
You don’t have to eat to the insulin nor eat if you don’t want to, once you’ve been taught you’ll be able to do corrections too.

I understand you don’t want even more injections, but you have coped with the ones you’re doing now haven’t you.

The people here helped me a lot when I went on MDI , we’ll help you too.

Hi Simon...I can't offer you any advice on your insulin regime...still woefully ignorant of insulin use...however...you clearly need some support from your surgery...don't view it as complaining...asking for support is necessary ...whether your surgery is short staffed due to annual leave...new parenting or study leave...there is no excuse for such indifference/shabby treatment...I agree with the other members here...you have to be assertive...I would urge you to call the practice...insist you need to see either the GP/DSN (or at least speak to them again)...are you able to write a quick letter/email to the practice manager...there is no excuse for cancelling a review without a follow up appointment...it is hard work trying to establish a good working relationship with your health care team...had difficulties myself...however if we don't assert ourselves...we often get overlooked...ignored...give them a call...let us know how you get on...good luck.
