Like Mikey B I was not able to answer the first question ( if you use multiple choice for a scale you need to choose options that have no gaps and no overlaps). However I continued and went for the nearest group.

The questions about T2 were irrelevant as I already have T1. Perhaps you could include an option, if you have T1 skip the section on T2 (got end of survey)

The questions that you have included about T2 make a lot of assumptions about the condition which have been identified by others.

I am not sure what A level Course you are diong but think it would be well worth you getting your tutor or a friend who is diong Maths to look at the structure of your questions, and have another go, as there are lot of people on here who will be only too willing to help.
 
Oh the other thing is, you can't tick anything to signify you imbibe less than one alcoholic drink per week - so I had to say I have none at all, which is a fib. In general health terms I suppose I'm less likely to have 'alcohol induced' conditions - but I don't like any questionnaire which forces me to answer incorrectly.

I also don't understand why you need to know how much a person drinks! - as far as I'm aware no amount of it causes ANY type of diabetes?
