hi, I'm new to this forum, my daughter was diagnosed last August age six, she's on her first residential in a few weeks, our nurse has said there is a care plan we can use on here......please could someone help?
 
Do you have a care plan at school? If so you could just adapt that one and write it yourself. It needs to include basically the following:

How to deal with hypos (especially if they are visiting somewhere at the time, you don't want your daughter to be expected to carry on walking around while she's low)
How to deal with highs (and exactly how high are you happy for her to go before something needs to be done about it)
Mealtimes - working out her boluses and doing them
Evening dose (is she on injections or a pump?)
Do you want her to be tested during the night, if so how often
Emergency phone numbers
Are you sending her with a glucagon kit, if so staff will need instructions on when and how to use it
Step by step instructions on how to use her test kit and any other equipment she will have with her
And anything else you can think of that they might need to know, write it all very clearly so there can be no misunderstanding. I always think it's better to send too much information than not enough, but it needs to be clearly organised into different sections so that if your daughter is having a hypo for example the staff can look through and find the relevant heading quickly.

Also find out which teachers and first aiders are going on the trip and try to arrange a meeting with some of them so that you can go through the care plan with them and make absolutely sure that they understand it all.

Hi @Lisaizzy - welcome to the forum. Yes, there are quite a few resources available to download on our website under School Resources.

If you look at the Residential School Trips page, you will find the Residential School Trips tool. It's got space to record lots of information for school staff and includes a checklist of what is needed before the trip and during - so could help you with recording and answering some of the questions you need to answer - as Sally has mentioned 🙂 .
You can also find a sample copy of an IHP - Individual Health Care Plan on the school resources page - in case that was what your daughter's nurse was referring too?

Hope your daughter has a great trip. Let us know if you are having any problems. Does the school offer good support at the moment?
