The DSN from the community diabetes team at the hospital who supports me has been on leave for 2 weeks & as promised I've put my BG'S together & emailed them to her. If anybody could give me any feedback before she replies to me in the next day or so it would be interesting to see what others say/think. Tia

Hi Gill, there's certainly a lot of information there for the DSN to work on 🙂 The notes explaining the 'out-of-range' numbers show that you are thinking about reasons for the levels, which is what you need to do - and try to remember for the future. The more you realise things go wrong when you forget to bolus for something, for example, the more likely it is to stick in your mind and make you stop and think the next time 🙂

So the DSN said "it looks like your BG readings are held from going to bed to waking which would suggest in theory that your dose of Levemir at night is correct. However, the readings during the night show that generally your readings tend to drop"

Here's what she has asked me "I wonder how you feel about perhaps increasing your ratio with your evening meal but dropping your dose of Levemir at night?" ~ I'm waiting for a reply for what to alter the 2 things to

Usually, the advice would be to change only one thing at a time, so you know the effect of that one particular change - if you change two things it might not be clear which might need further tweaking if the changes don't have the desired effect. You might want to question the DSN about this 🙂

So here's what the DSN suggested changing the 2 things to
I would be tempted to try 1.5 units of insulin to 10g ~ The ratio change is in line with DAFNE but if you use an Expert bolus advisor, you would have the option of 1:11g or 1:12g, etc. It may be a trial and error change.
and reduce the Levemir at night by 2 units.
again this may be too much but it should be a safe trial and can be adjusted from there.
I decided to call her and question about changing 2 things at once & she said to do whatever I feel comfortable doing ie if I would prefer to do just one thing at a time & see how things go...... The DSN said she would be tempted to change the levemir before trying the ratio change ~ this is what I'm going to do & maybe give it a week from now to see what happens

I also asked about the times of taking Levemir ~ The general advice is to take the Levemir before bed - there appears to be little evidence of a benefit to keeping the injections 12 hours apart however, there appears to be little evidence against doing this. I would be swayed by your own person preference, if you are likely to forget before bed then earlier is fine, but if you find it restrictive taking it earlier, I would be tempted to take it before bed.

Tonight will be the first night of changing my levemir from 8iu to 6iu so we'll see what happens...
