Being in a mh nursing home please read if you can

Hi i live in a mental health nursing home- soon to be moving onto independant living. I am on a basal bolus regime. I take novorapid 3x a day with meals and togeo.
So i am now on 16u with breakfast 18u with lunch and 18u with dinner.
I checked my sugars they were 9.4 and i said to the nurse ill just take 10u novorapid as my sugars(glucose) is lower than it has been for along time. He said no you have to take 18units thats what is prescribed so you have to take 18units i tried to explain that the diabetic nurse said if my sugars are lower i should take less but he didnt believe me as usual so ive had 18units now and what they had on offer for food was pastrie pie things and ive never liked them so i just had some toast. Im posting here because i dont have anywhere else to turn to so im sorry if this post upsets anyone. Im so scared ill go into a hypo ive never had one and i was diagnosed in december type 1. My long acting insulin is togeo and i take 60units at night.

I guess from writing this post i was hoping to get some advise if possible and if not thats ok too. Im just so anxious all the time and its really affecting my mental state to say the least. Thanks for listening.
 
I suggest that you ask for someone in authority to come and read what people managing their blood glucose using insulin will probably write here in response to your post. I can't imagine that it will be anything complimentary about the care you are receiving, but people need to be educated about the use of insulin and the consequences of it being forced upon someone who is adjusting their requirements as appropriate.
They do realise that hypos are serious - actually - they probably don't have a clue.
Congratulations on managing your levels so far, by the way. As a type two I can live without medication if I manage my diet - to have numpties insisting that they know better than you must be so galling after your success to date.
 
Thank you, my sugars since april have been consistently high until this evening. I was shocked and pleased but being forced to take that amount of insulin and disregarding my sugar levels is a scary thought for me. The nurse is an rgn and he even said "you are scared the insulin is too high" and i said yes i shouldnt be taking that much but as i say he said take the 18units so i had no choice. You know i just feel like bursting into tears and i cant wait to move from this home. Thank you for replying.

How are things now? Have you been told how to balance the carbohydrate in meals with your doses? If you eat the amount of carbohydrate that your insulin dose 'needs' then you should return to the 9.4 that you were before eating after 4-5 hours.

But I'm a little worried that you may not have had as much carbohydrate as you were expecting because you didn't like the food.

How much toast did you have?

Do keep checking your BG levels regularly through the evening.

It does sound rather like the people looking after you need a few more pointers about insulin management, dose adjustment and carbohydrates. Can you get your diabetes nurse to put some basic notes in your file?

Hi my BG Levels when I last checked were 13. I had 2 slices of toast- symptom wise I am profusely sweating and a little light headed! had my Toujeo 60units at 9pm.

im going to check my BG levels now and see what they are.

Well it sounds like it was just as well that you didn't reduce your insulin since your readings would probably have gone much higher if you had, so you must have managed breakfast well despite your concerns.
I would be surprised if the sweating and light headedness is down to a reading of 13 or 12, but it may be due to anxiety because it sounds like you are quite stressed, so do try to relax. Do you know any meditation or breathing techniques? Stress/anxiety will usually push your BG levels higher, so it is best kept under control as much as possible.

Sounds like you are doing really well with your diabetes especially as you are still relatively newly diagnosed.
