Annoying survey pop up?

Seems to have gone now but earlier it was popping up here there & everywhere, hope its not going to be regular occurance.
 
Yep, but only on my laptop, nothing so far on my iPad. Is it a scam?

I did it. Hopefully it’ll be useful, because I can never find my way round the DUK website. It was a series of exercises where you had to navigate through screens of options to get to various parts of the website. eg, 'you want to find out what tech is available for Type 1s' or 'You want to find a group in your area' etc. I had to abandon one task, I couldn’t find the relevant page anywhere!
