Good evening all, hope you are well.

Got my results back from my latest check up. HbA1c was 6.1 or 43. Cholesterol was 2.6 and everything else was absolutely fine according to my consultant.

For background I am 32, type 1 for about 16 years and use lantus (split dose morning/evening) and Novo rapid. 6ft 2 16 stone 12, very active, exercise 5 days a week at an intense level.

I have however had an issue with a raised ACR (urine albumin to creatine ratio). I believe this is meant to below 3 mg/mmol and I was sitting about 4-6 and was on 5mg lisnopril. The numbers remaind steady for the last couple of years but recently increased to 7 so I was placed on 10mg last year. This time my result was 30!!!

I did have a very hard 45 min high intensity training session consisting of weights running before giving the urine sample and my consultant said this could have been a factor for it to be so raised.

I have to re do the urine test and have my BP checked. BP has usually always been fine around 125/75.

The consultant didn't seem to be overly concerned, however has anyone else experienced this with ACR and exercise or persistent raised ACR, any comments would be appreciated.

Just a question I’m intrigued does splitting the lantus work well . I was led to believe it has a 24 hr profile so didn’t need splitting

Yes Freddie it does for me, i believe most people are single dose users. I find it more flexible with my training routine and can finely tune my dosage a bit better depending on what I intend to do training wise. However if you were considering it, I would speak with your specialist.

I tried splitting Lantus and couldn’t get it to work for me @Freddie1966 - so I guess it varies - we certainly have others on the forum for whom split Lantus works well.

In the end the most success I had was taking Lantus at breakfast.

Good to hear that the consultant isn’t overly concerned at this stage. The ACR is connected to microalbumin and kidney health. I don’t know how scientifically accurate it is, but it was once explained to me that the kidneys are like very fine sieves that filter the blood, but over time with diabetes small tears and holes can develop which allow small fragments of protein to leak through.

Keeping blood pressure down can help I believe?

Yes I believe that is correct with the BP, however my BP has always been normal and I was originally on Lisnopril 5mg then 10mg to try and keep the ACR down and not lower the BP if you get me? Fingers crossed it's just a blip and if isn't, we will just have to go back to th drawing board and work out another plan.
 
Urine results back and ACR was 3.1 so just 0.1 above the normal level which is a relief! consultant believes the high number of 30 was down to post exercise exertion and dehydration potentially.

Nurse took my blood pressure and the cuff was too small for my arm and came back at 139/76. I told her it was too tight and she even had to hold the cuff together as it was tightening. She re did it on a different setting and level was 114/67.

Seems the increased dose of 10mg lisnopril is working as it has taken the level down.
 
Don't see how she could have been a nurse if she did that with the BP cuff. I don't currently know a single nurse (personally) yet my entire family (other than the ones under c.15) can happily and competently take each others BP.
