A Guide to filling in your Diab1 Medical form

Q1b (inc Gliclazide) says to go to Q3 when completed but before Q3 it asks " Do you check your blood sugar levels ?" I intend not to complete that but answer "Yes" to Q3 and Q7. I guess DVLA only cares about blood sugar checks related to driving.
If you take insulin you complete Q1a and Q2 and Q3. If you don’t take insulin you don’t need to be filling in the DIAB1 form unless your doctor/nurse advises you to report to dvla or you’d like to report it. If you don’t take insulin 1b says go to Q3 so you don’t fill in Q2.

What’s the point of printing it? Why not submit it online. You’ll get your licence far quicker.
I think that’s what the OP intends to do, but like many of us would like a copy for his own records

But why? Just save the online application if you must have a copy of what you said.

I've had some iffy experiences with electronic submission of important documents. Usually a matter of incompatible formats. Next time (3 years) I'll probably go all electronic. Not having to write it out is a major benefit anyway as it's been a few years since I had to pay attention to not making written mistakes. Having an instant means of correcting and erasing errors encourages bad habits in writing.

Exactly. I'm going to hand sign the original and email the lot as a series of .pdf scans. I once emailed important stuff to an agency only to find later they don't take jpegs so after a month I resubmitted them as .pdf files. The price of experience !

@Busdriver60 has not heard anything from the DVLA regarding the Group 2 DIAB form he sent out to DVLA on 20 May 2024, following his Diabetes diagnosis earlier that month. Should he need to chase them up for any update? Advice please...

Might be worth dropping them a line to check on progress. Though it can be a bit tricky to get through.

Yes, I would. I submitted my DIAB for Gliclazide after June18 and got a positive reply last week. So quite quick.

Hi

I am just starting to use insulin so need to inform the DVLA and am having trouble filling in the online form. The online form asks if I have seen a Healthcare Professional about diabetes in the last year to which the answer is yes. This then takes me to a page to enter the details of my consultant but I don't have one as I see the diabetic nurse at gp surgery. If I enter 'no' it takes me to page for entering GP details. Should I say no even though this is not true? When I got to the page at the end when I had to tick to say everything was true it said they didn't have all the details & started to take me back through the form. Want to scream now!!
Maybe I should just fill in a paper form?
