I was about halfway through a cartridge

I fitted a new needle; dialled up the dose; pushed the needle into my belly; then pressed the slider
At that point the dial/knob should have rotated and the plunger should have moved down to dispense the dose - but nothing

Tried again, still nothing
I took the two components apart; dialled the dose; pressed the slider so the piston was in the open air, and it worked perfectly
Tried it all connected up again; fiddled and twisted, but still no joy

Eventually I fitted a new needle and everything worked perfectly
I can only assume that the first needle had become damaged or blocked somehow, and because the insulin couldn't get out, the piston couldn't go down

Oh well, I hope I remember that if it happens again

This happened to me a few times when on mdi, duff needles. I used to do a few units air shot before injecting, to check needle was functional.

I always 'air shot' half a unit to make sure insulin is ready for injection.
If you don't 'air shot' how can you be sure?

Well - you can't be sure and that's why I did it every single time whether they were bolus, correction or basal jabs. Cos you HAVE to. End of story!
