4 days of highs, changed pump, changed insulin

Hi Folks,

I am sorry I am just having a moan.

All was all going well, I had a week ranging form 60-80 TIR each day, 0% low or lower, and zero high alarms (higher than goal, but not high). Thursday it started to go wonky.

Pod change Thursday afternoon, was working then, but then overnight high, jumping high after meals, and not impacted by exercise.
Then changed again yesterday morning, and I gave up with that one giving it time to work, as the one before didn't.

Saturday and yesterday I just couldn't get in range at all, and now I've reach that point where I have all the high symptoms, including difficulty concreating and thinking straight, tired, weeing. I just want to cry now. I just feel so fed up.

I thought it was my last pump not working as well, then I thought with the new one on, maybe it didn't pick the best absorption site. Now since 10am) I have the pod on my arm which is a tried and trusted site, and a fresh bottle of insulin, out the fridge for 12ish hours, I suppose it is cause of just waiting for it to back into range, 14.1 now and going down, but its been between 13-17 since 7am and I feel so gross, sick and tired.

I am really fed up with the HCL doing fine for weeks, and then just falling off the planet, and it is never clear if it is the pods, or the HCL.
I thought it was supposed to learn, but it seems so slow at learning. That last pod defiantly had some insulin on the surface of my skin, as did the one before. I do think it is a downside of the patch pump, not being able to see the site, that window shows if the cannula is in, if there's a bad leak of blood or insulin you can see liquid but if its just a slight leak there's no sign.

As per the graphs, on both pods it does comeback down at times but then be high around meals, exercise seems to do nothing at all, seems like I am peaking at meal times, but my carb ratio has been the same for months, I really hope it hasn't suddenly just changed for no reason. Rugby only stopped last week, I hope it isn't that, as I have still been walking as much, if not more and I can't exercise if I feel like this. It's annoying as if I cannot think straight I do struggle to see the patterns because my logical skills just go if I have high levels for more than a few hours.

Could be my period as that started Thursday, I need a clear mind so I can look at all the data. Think I need an appointment with my nurse, when I have looked at it.

Sorry this is mainly a moan.

Glad to read that you switched insulin vial. I had a couple of days of highs and just didn’t think of changing that, until the light dawned!!!!!

I don’t use the same system as you so I am not sure whether this also applies to yours but I find that the HCL is good for things in the middle but not so good when my levels have gone too high. In that case I come out of looping and use a temp basal and also increased any corrections, as I would have done before looping. My increases varied from 120% to 150% depending by just how high I am. When that didn’t work recently I finally cottoned on to the problem being the insulin. Can you switch to manual to bring things down?

There are also other times when it is just Sod’s Law!!!!
I hope that you get things back in range and that the next pod behaves.

I was at 12.3 when I bolused for lunch, I thought give it a little longer before you eat, waited 15 minutes, was at 11.4 and slow downwards, so I ate, it seems to have then paused my insulin for 20 minutes and now an hour after eating I am back at 14.

Maybe I go into manual, I feel like having a correction now, and it is telling me not to, I haven't been able to do much work as my head is cloudy and I won't be able to play touch rugby later.
 
I don’t loop @PhoebeC but I couldn’t put up with that! I’d be overriding the loop and getting it sorted personally. With the loop, it’s hard to tell what’s causing the highs. It sounds like a site issue, but with the loop ‘helping’ I find it hard to pick out what’s happening and whether your new site is ok.
 
Frankly I have battled with the Omnipod HCL since last June. I have found it totally hopeless. I switched it to manual some months ago but I still feel that in some way it is limiting my ability to manage my BGs in manual mode. According to my DSN a number of people I started on the system with have also ditched it. I asked to go back onto the Dash but I was told I couldn't. On the other hand I was offered the Ypsomed with CamAPS. I am transferring to it on Thursday. Just simply doing the online training and seeing how much thought has been put into CamAPS as opposed to the simplistic software of the Omnipod 5 I am very impressed so far.

@PhoebeC it might be worth asking if you could possibly be offered something else.
 
I set into manual mode and did 2 corrections using Dafne and also set up a higher basal. Can’t have that on in loop mode.

That got me back in range, I didn’t know the carbs at tea and it did drop nearly into a hypo, but I’ve kept it in range and it’s been in range now for nearly 6 hours.

@PattiEvans I loved the Omnipod dash actually, it wasn’t perfect but I was getting there with my rugby and periods. I don’t think this will ever get those things. I’m due my bloods with my GP so will get those booked in and get actual results I can use in support. Got my consultant appointment in July, will speak with my DSN in advance about options.
Happily go back on the dash. Urgh
 
Much better since yesterday afternoon, did try and go high again this morning but I’ve beaten that’s.

It’s annoying as the same effort went into the management Friday - Sunday and that time was 29%-43% TIR, and most in very high, and now today is back at 77%. Effort and reward with diabetes is never equal.

Sorry to hear about your O5 woes. It certainly doesn't seem to learn like it's supposed to. I feel I'm putting more effort into the HCL than I did with the Dash!

I agree with @nonethewiser - a DSN appointment to discuss its shortcomings. You / We deserve better.
