Boxes on the bottom of the DIAB1 form..

Me and Mum are currently at a stalemate as to whether you are supposed to fill in the boxes right at the bottom of the page (the name, DOB, ref, driver number boxes) I think it looks like its something they fill in - after all how the hell do I know my reference number? Mum thinks you have to fill it in incase the pages get separated... Does anyone know whether you are supposed to fill them in???

Also I already have a driving licence so do I just send them the form and they will see my driver number and go from there? I don't need to send anything else do I?

I just Googled this as I've got my renewal coming up soon. I found a Youtube guide where it demonstrated someone filling in the form while talking you through it. You could see those boxes on the film, but they weren't mentioned, and weren't filled in. So I assume they're for 'office use' . My last form, in 2013, didn't have them on the form at all.

I also don't have a consultant yet - I presume its going to be ok to leave it blank then send them an updated form once I get a consultant (might write currently n/a in the top box or something) I haven't ticked that I see a consultant on the following page so surely they will realise from that :confused:

I left that blank last time, because I'd just been referred back to my GP surgery nurse. I just put 'referred back to GP' or something in the box. It didn't seem to cause them any problems, they just need to know the best place to get any extra info they need.
 
Thanks, think I will put N/A not currently under one or under specialist nurse or something! I will be having appointments with one but not got there yet! And I don't actually know who my GP is! The one that has signed my prescriptions I've never met so I'm just going to put the one that sent me to hospital at least the surgery address will be correct! (I hate forms!)
 
For your GP you can put the 'lead' doctors name - years ago I was at Northumberland House surgery - aka Drs Ball, Herbert, Hickman & Jethwa - so Dr Ball, Northumberland House surgery was the answer.

Now it's also a surgery name, and the 'lead' Dr has recently changed (cos the original retired) but he happens to be the one 'with a special interest in diabetes' anyway, so any forms would be automatically referred straight to him, and that's exactly what will happen if they need to know anything about you - they'll have a look and see who happened to deal with you as you are new - if he's not the one 'with the interest, they will liaise - and whatever is asked will get answered, so don't worry! If there are any queries on it at the docs - they will ring you.

Because I get most of my treatment at the hosp but for some reason they asked the GP to complete the form - Dr E was on the phone to me for about 15 minutes cos he didn't know most of the answers - but he believed me and he knew it was quicker to do that than try and speak to a mega busy consultant at the mega busy clinic at the mega busy hospital LOL

They are as stupid incidentally with the doctors as they are with us - one question on a lorry drivers form (they have to have medicals to keep the licence to do the job) was 'Is the person's BP less than 140/70? Y/N' - he'd answered 'Yes - 119/55' - thinking they'd think 'Great' the same as he and the lorry driver did. Instead they wrote back and told him he hadn't answered the damn question - since he was only allowed to put Y or N - and he hadn't .......
 
I have just filled in my forms and for consultant there was none as GP is the one I go to. So section left blank.
I can not remember filling in any bits about ref numbers so bottom of page put your driving licence number on it to be safe. Did you download the form or did DVLA send you the form if they sent it with a covering letter it might have a ref number on it. If the section says official use leave it blank otherwise fill in DOB licence number etc as requested.
 
I'm doing it myself I haven't been contacted by the dvla to do it (newly diagnosed so previously had nothing to report!) I've put n/a not currently under one in the consultant box and then not filled in the bottom boxes because they don't mention it in the guide to filling it out - I figure at worst they will end up contacting me saying they only have half a form!
 
If it's asking for your driving licence no, then you need to put that in the box as it corresponds with your licence and they can match you up. If you don't fill it in properly all it does is P them off no end and they will start sending the forms back and asking more questions. If a question is asked answer it. If not asked don't give any information.
 
@EmmyBuzz I have just looked online at the form, Yes you do need to put your Name, DOB and driving licence number. This is normal for all forms. Obviously if you have no Ref no, then leave that blank.

I think the confusion has arisen partly because of the DVLA YouTube video, showing how to fill it in, which shows the boxes at the bottom, but makes no mention of them when showing the form being filled in, either on the screen or in the commentary. Obviously it can't hurt to put in the info, if known.
 
It is so that should the pages get separated at their end, they can be allocated to you otherwise they could refer to anybody! (sorry I'm late replying, it's only just struck me that I knew the answer)
 
I've already sent it without filling it in - they make no reference to it in their own how to fill in thing as far as I could see - hopefully it wont cause problems but knowing my luck it will :(

Let us know what happens, I've got mine to renew later on this year!
 
I will, I'm not really sure what to expect I guess they assess your answers and maybe contact the doctors to confirm things? I presume they will contact me or send me a new licence? I only have about a year on mine before its due to be renewed anyway I think!
 
Hi I've just started on insulin and have just received back my medical restricted 3 year licence. I did not fill in those boxes at the bottom of the form and there was no query with that. I am under care of my gp so I filled in those details but left hospital and consultant boxes blank and agin no query.
There is a space at the top of the page for you to put your drivers number
