Hi guys have other members found changing from novorapid trurapi that there having to take extra after four hours or is it just me.

I've never used novo as I was put straight onto trurapi. But it seems to last about three hours. If I've eaten a lot of protein I often have to jab a couple of extra units then to stop the steady drift upwards.

Welcome @marc89474 🙂 Some people have mentioned issues with Trurapi here. However, have you done a basal check? Rising blood sugar after 4 hours can sometimes be due to a basal issue. What basal insulin do you take? How long have you had Type 1?

Which basal insulin do you use?

If your basal insulin dose is not optimized to keep your levels reasonably stable in the absence of food and bolus insulin then it is tantamount to building on wonky foundations and things will become more unstable and unpredictable when you add in food and meal time insulin. Most of us find that we need to adjust our basal insulin dose(s) from time to time to factor in things like more or less exercise or illness or change of the seasons.... Quite a few people find they need less basal insulin in the summer and a bit more in the winter.

Lots of things impact our insulin needs beyond just food. Exercise, illness, stress, hormones, alcohol, ambient temperature, medication like steroids or sometimes even statins.

Can I ask, if you are not carb counting, then how do you manage your diabetes and keep in range? Are you just on fixed doses and if so, do you keep your meals roughly the same size each day?

Control is very hard without carb counting. It’s a pet peeve of mine that some Type 1s don’t get taught it at diagnosis. It’s not hard and you don’t have to do a DAFNE course. You can check out this online course if you like:

You didn’t say which basal insulin you use? That might make a difference. You can do a basal test gradually over a few days:

If your basal issue wrong, it makes it very hard to get everything else right. It’s like the foundation of a house: you can’t build a strong house on weak foundations.

Finally, if you think the Trurapi is contributing to making things worse and isn’t suiting you, then ask to be swapped back to Novorapid. They shouldn’t have changed it without your agreement.
