Spiking and hypos since using libre

Hi all. Thought I'd give you an update on where I am. I feel like I have achieved so much since last night and feel so much happier and assured in what I am doing.
I made the decision to swap back to humalog before dinner last night. No hypos and remained between 4 and 7 mmol/l until morning.
Took humalog straight away to combat the dawn phenomenon and checked bloods every 5 mins to see when they began to fall. 50 mins later I was able to eat my breakfast. Readings only climbed to 10mmol/l. So I did my normal exercise, 3 hours after breakfast and snacked on a banana beforehand. No hypos during or after like I was experiencing on fiasp, yippee. Decided to do a basal test so skipped lunch and all afternoon my bloods have been between 5-6 mmol/l. So I can safely say my basal amount is correct.
I therefore blame this troublesome experience on fisap. It lasted too long in my system, causing me to hypo and it just didn't agree with me.
I have learnt though to take my insulin as soon as I get up and to delay eating until my bloods are reducing. So thanks for all your help and advice everyone. I think I'll be ok and can't wait to be rewarded by a lower hba1c result ☺️

I wouldn't say you've done a basal test fully as a complete basal test takes a few days to conduct broken into different time blocks rather than just a few hours in one day but well done for todays results 🙂 xx

Ok thanks. What time block would you recommend next? I haven't eaten anything since my breakfast at 8.40, apart from a banana to get me through my exercise and it's now 6.40pm and bloods are still in recommended range.

That is great news. So pleased the prebolusing worked to flatten out the spike and you are back to an insulin you are confident with and suits you. Shame the DSN didn't suggest prebolusing earlier and save you having to mess around with Fiasp in the first place.
Pleased to report I've had a really stable BG day today too. Just 3 units of Fiasp this morning with 15 units of Levemir and no more insulin the rest of the day. Can happily cope with just 2 injections a day!

Thanks. Glad to hear your day was as good as mine

Hi there,
interesting the Fiaspo you went thru.

>>Maybe there a page on diab.org.uk which presents pictures of insulin activity profiles over time side by side?

I had similar issues when I swapped from animal to human insulin years ago. I think the animal insulin was a lower concentration e.g U60 cf the human U100 only. This meant for a child especially, less titratability of the U100 cf the U60 vs correction requirement. Whether the human vs animal sequence had any effect is difficult to separate from the conc diffs, although the animal short acting insulin seemed to have a longer lasting effect cf the human short acting (Actrapid) which maybe provided some HbA1c benefit. Since swapping to U100 hypos have been more frequent and severe.

Re predosing, one can come a cropper if anything interrupts the planned eating e.g. a lengthy tel call, just finishing a physical task eg gardening, cleaning out the garage, vacuuming; a film which is engrossing; a demand from work or just forgetting. There a lotta distractions. So its a pity one cant note on the their test meter that one has predosed and set a reminder alarm to scoff eg 20 or X mins later. For now, I just set my egg timer

Hi. Are you not inclined to swap back to prevent the frequency and severity of your hypos?
Due to covid I'm working from home so the pre bolus method is easy atm as I sit at my desk after injecting and remember to check bloods before eating. Going back to the office will be whole a new ball game as I'll have to change my routine and ratios for sure.
I use the app on my phone so if need be I could always set an alarm on there . Love the egg timer

Totally agree with you @daducky88 Was just thinking this morning that having an adjustable timer option on Libre for when you input bolus insulin as well as for food would be helpful..... I had a change to my routine this morning and started the day higher than usual so I needed longer to prebolus and got distracted and suddenly felt wobbly at which point Libre showed me 5.2 with a downward vertical arrow... 1hr and 22 mins after injecting my Fiasp!!
You would also think with the food timer on the Libre would have a 30min reminder option to check after a hypo... ie 15 mins for glucose to hit the blood stream plus 15 min delay to get into interstitial fluid. After all it is set up to give you 15g carbs for hypo treatment presumably, so you would think it would have a hypo retest reminder timer.

Actually I've just remembered, when I was low the other night I clicked on the hazard warning and it gave me an option of a 15 minute reminder to check bloods again. It beeped after the 15 minutes so I could check as again. Hope you get find this on your device.

Do you use an app on your phone or the Libre reader? I only have the reader and not seen that option.... just 1hr or 2hrs on the food input page.

I use both, mainly phone, but libre reader during night if I have hypo, as phone switched off. Hope you find it.

Guess I should probably dig out the instruction manual and look it up, rather than try to find it next time I am hypo!

Found it thanks!

I see that link says 5 hours activity time for Fiasp, the same as NR which is what I thought..... But then it suggests you take it 2 mins before food which would be useless for me. It needs at least 15 mins pre bolus for me for afternoon and evening but in the morning usually about 45mins, so that info isn't exactly perfect either!
