Really sad. So easy to prevent in hindsight but so hard to predict in reality.

That is very sad and must have been awful for the children

An Addison's crisis could not have been the cause of the problem though as a crisis involves very low blood sugar and in crisis it is imposisible to go DKA. You need to be pumped full of steroids to raise the bs and BP.
