What type…..that is a question

Hi All

I posted on here in January 24 when my brother was admitted to hospital with blood sugar of 33.6 and A1C of 145! He was prescribed Metformin 1000mg and was put on mixed insulin 70/30 (Humlin)

Roll on 6 months and he is still on the mixed insulin but significantly lowered his insulin intake and last A1C 48 but still not been diagnosed type 1 or 2. The DN has now referred him to the Diabetic clinic at the Hospital as she could not interpret the blood results.

My brother does not ask questions so I am going to the next appointment. This was on a recent letter from the hospital:

IA2 antibodies 18.5 High
Antithrombin 71 slightly low
c-peptide 2408 high

From my googling the c- peptide suggests he is still producing alot of insulin but the IA2 suggest type 1, so does this feel like LADA?


To be honest I have no idea what I am talking about I just want to be prepared to ask the right questions at the appointment

Any advice I am grateful for
 
It doesn’t feel like any type in particular because high cpeptide points to type 2 and high antibodies points to type 1. Usually you need multiple antibodies for t1 though and the cpeptide is extremely high (>600 after a meal points to t2 and his is 4 times that).

For context my cpeptide was 235 after a meal with a blood sugar of 18 and that was considered too high for a definite t1 diagnosis. His is 10 times as high.

So I’d guess a strong chance of t2 but that he likely won’t be given a firm type yet. I’m 15 years in and don’t have a type yet.
 
I agree with @Lucyr That’s a high C Peptide. I have IA2 antibodies and I’m Type 1, but I also have another Type 1 antibody. My consultant said that two or more antibodies is likely to be Type 1. I hope it’s not a rude question but is your brother overweight?
 
Hope your brother gets some clarity over his diabetes type @GaynaChick

There do seem to be quite a few people with unusual or atypical presentations of diabetes. Such a high cPeptide and still needing to inject additional insulin seems indicate quite a bit of insulin resistance.

Do you know if the cPeptide has been rechecked to ensure it wasn’t a rogue result?
