What kind of help can I get?

Since I've moved to the UK from abroad I really don't know what kind of help I am entitled to at all.

At the moment the nurse at my GP's practise is doing a great job. She obviously knows what she's talking about, but she doesn't seem to be a specialist on diabetes.
I've had great help in the Netherlands (where I come from): a specialist diabetes nurse and a dietrician. Therefore I know quite a bit already.

The only thing I can't seem to do is loose weight. I'd like to try and loose weight, but prefer to have someone help me with it. I could do with extra info and monitoring.

So...what am I entitled to and where can I get help?
