Web link please for reporting Libre sensor failures

I've read every word of the Libre website this morning to try and find the email form - or phone number - and can't. I do have 'an account' since I have Libreview - but why on earth would I need to register again to report a failure? esp as the site says 'if you need to access the shop' - which I don't? Does it even become apparent how to report when you do register again?
 
I have never used the online form but I am sure @Lucyr will have a link to it.

I have only ever rung the customer services line and the number for that is on the outside of the Libre boxes, printed on one end. I think there are 3 or 4 numbers depending upon which country you are calling from but it is pretty straight forward..... Hang on and I will get my glasses and get the number....

I don't think the customer service lines are open on Sundays though.
 
Tells me to ring rather than email, cos the email link automatically goes to Outlook and we haven't been able to use that method of email access for years - have to access them via Virgin Media's website since they're V's email addresses. Hey ho - will ring em.
 
I ended up ringing them when I had a problem and it was a long and tedious experience. The online form is so limited it's almost useless but there is the option to email them if you're not in a hurry.
 
I have an email address for Libre support in Ireland, but after I reported a couple of sensors as faulty they sent me back an email which included this text:

In the future, for queries such as sensor fall-offs or error message/s while scanning, you can complete our online sensor support form at a time that’s convenient for you. Once submitted our colleagues will be in touch to help.

I don't have an email address for UK support. I haven't had need to contact support since that time so I don't know if this form link is more or less helpful than the email address I have for Abbott Ireland.
 
Well, I think you have jinxed me Jenny! Having not had a problem with a Libre sensor for well over a year, my current one died last night with 2 days to go. Woke just after 4am slightly hypo I think (didn't finger prick as didn't have my test kit to hand, so just ate 1JB) and went to scan and my reader said sensor had ended. I got my phone out in case the reader was the problem but no, it also said that there was an error and I needed to replace sensor! Oh well, that will hopefully give me a buffer of 12 days if they replace it as I have been a bit "hand to mouth" with my Libre prescriptions recently.
 
A quick 5 min phone call for me to the helpline and a new sensor is on the way. I do struggle to understand the foreign accents but we made it through all the questions with only a few repeats necessary and the call taker was very pleasant and polite.
