I take my tresiba at 7am ish but up to half an hour either side - hope that's ok - if anyone knows please let me know

How would you go about moving the dose time to 10pm at night?

If you wanted to move it to 10pm just wait until 10pm to take it. You can do that with tresiba. I did exactly that (moved from around 7am to 10pm) 2 days ago because I wasn't sure I had taken my morning dose. As long as there is at least 8 hours between doses, it wouldn't of made any difference as a "one off" change whether I had or hadn't taken my morning dose. I wouldn't think one would do this often as it might cause problems, but occasionally it's never been a problem for me and it's within the technical advice on the tresiba pamphlet.

Thank you. Also I emailed my DSN to ask her and she says it's usually recommended that you move it forward by 2 hours each day until you get to the time you want to take it

I think if that is the advice you have been given, I'd stick with that. Just avoids any issues if they ask why hadn't you taken our advice. I can only work from what the official advice says and what works for me personally.

I've always been advised to take my basal at night, whether it was the Lantus previously or the Tresiba as now

Yes that’s what ours suggested too. And if you usually take it at night and want to move to the morning then two hours earlier each day instead. Works for travel if you change time zones too.

Anywhere within a 2 hour window is fine for your daily dose too. People have lives and not everything is the same every day but doing your basal at a time where it’s likely to be consistent helps.

Why are considering changing the timing @mum2westiesGill ?

What are you hoping to achieve?

With an insulin that lasts as long as tresiba, I can’t imagine the time of day you take it makes much difference to blood sugars, so the best time to take it is probably just the easiest time to remember.

Someone mentioned on another thread of mine that maybe a change of time of when I take it might make a change to my bgl levels.
I might leave it because as @Lucyr said in her reply "With an insulin that lasts as long as tresiba, I can’t imagine the time of day you take it makes much difference to blood sugars"
What do you think @everydayupsanddowns ?

Someone mentioned on another thread of mine that basal needs can change

Yes basal needs can change. But I think that meant the dose not the timing?

My understanding is that one of the things about Tresiba’s long action (over 24 hours) is that there is more flexibility over dose timing, and that daily doses are likely to overlap somewhat to provide a more consistent, even basal dose through the full 24hrs.

I’m not sure changing of the timing of the dose of Tresiba would have as much effect as some other basal insulins?

Many have changed from morning to evening with their Tresiba and found it worked better for them, also possibly by moving the timing it could avoid morning lows which may happen at other times of the day instead BUT in this case if a pattern formed with that the previous meal ratio could be altered to account for that possibly meaning they no longer happen, just my experience after almost 5 years of use xx

Having moved dose times of Tresiba myself this is how I did it and it worked for me. might not work for you. I would do a half dose at the 7am slot then at 10pm do a full dose. I went the opposite way and had no issues but appreciate the change overnight might cause an issue. you could also try a reduced dose at 10pm but more than the half dose

good luck
