The case of a three-year-old boy, who died after a catalogue of failngs by the NHS, has spurred promises of changes in the way complaints are investigated.

Sam Morrish died at the age of three, following a catalogue of fundamental errors by his local NHS providers and out-of-hours services.

He was vomiting strands of what appeared to be blood when staff at NHS Direct gave the case low priority and wrongly recorded crucial information.

When his mother contacted the local GP out-of-hours service, an unqualified call handler wrongly informed her that a nearby “treatment centre” would be able to help, when he should have been sent to Accident and Emergency.

Amid their grief, his parents Scott and Susanna, from Newton Abbot, Devon, tried to find out how the failings occurred so that no other child need die in similar circumstances.

But for more than three years they found themselves blocked at every turn.

Awful would be the understatement of the year 😡 Poor parents and poor little boy robbed of life.
