Authors report that “61.1% of patients with type 2 diabetes, but not healthy controls, exhibited aggravated insulin resistance towards the booster shots of the COVID-19 vaccine”.

Human volunteers were recruited to determine their immune responses and glucose control before and 2 weeks after the booster.

About 61.1% of diabetic subjects had impairment of insulin sensitivity according to the HOMA-IR index and about 66.7% of diabetic subjects had increased risks of cardiovascular complications according to the TyG index

The cause? Appears to be the spike protein in the mrna vaccines which can keep generating spike proteins for months after vaccination. Catching sarscov2 leading to Covid is also another source of the spike protein.

Given there is a reasonable chance of the virus being man made, we (still haven't) need to look into who, why, when, how, for what purpose etc.

I think Gain of Function experiments, which we know take place (and not just in Wuhan) are inherently risky. All it takes is for someone to inadvertently (and unknowingly) become infected through sloppy biosecurity, finish work, go home, mix with family and friends, go shopping (possibly at a wet market?) and you're in trouble.

From what I read there seems to be a growing shift away from a zoonotic origin to a laboratory origin, though the jury is still out. However, you'd be hard-pressed to find a better scenario for Occam's Razor.

You missed what the authors seemed to want to convey: the virus is also bad in this respect and they recommend mRNA vaccines as an effective way to mitigate lots of other bad effects. And (more specifically) metformin seemed to mitigate the insulin resistance, at least in mice.

They don't seem to have looked at how long this insulin resistance might last. (So in humans it looks like they looked at 2 weeks after vaccination, and in mice it looks like they were giving a booster once a week and looked at 4 weeks later. So neither seems particularly relevant for how vaccination might have any meaningful effect after a few months.)

So the message seems to be if you have diabetes, watch your BG after vaccination, and maybe ask for metformin if your BG rises and if you don't already take it. Though I'm not wholly sure how confident we can be of the results: this is a preprint, after all.
To me, the spike protein is something best avoided.
Indeed, we should care about ventilation, air filtering (including using masks), etc. And we shouldn't be encouraging infected children to be going to school (spreading the virus).

We only need to look at those things if it in fact was man made. So far that's looking to me like a (mostly) right wing conspiracy theory, partly to distract from measures to improve indoor air quality (and public health generally), partly to attack China, partly to attack scientists a bit (though likely they're mostly collateral damage, though there seems a genuine desire to attack the likes of Anthony Fauci).

I hadn't missed this, see my comment "Catching sarscov2 leading to Covid is also another source of the spike protein."
They don't seem to have looked at how long this insulin resistance might last.
What they have found is that the mRNA spike protein (aka as "The Jab") causes insulin resistance, total duration of which is unknown. Further studies will need to be done to confirm this.
So the message seems to be if you have diabetes, watch your BG after vaccination
The message is and always has been that the spike protein is dangerous. Manufacturing something which makes unquantified amounts of spike protein for unquantified duration (given batches vary considerably), seems quite dangerous, given the spike is inherently dangerous.
"To me, the spike protein is something best avoided." Indeed
So why use the spike protein part of the virus in "The Jabs" when there are other safer parts of the virus which can be used to infer immunity? Why use the dangerous part?
We should care about ventilation
Well the very opposite was true. Instead of encouraging us to be outside in the fresh air, they scared us all 24/7 and prevented us all from going outside for more than an hour a day (aka lockdowns etc).
We only need to look at those things if it in fact was man made.
Fauci (funder via the NAIAD of the Wuhan virus research) commissioned (which was then published) a counter lab leak paper. The author (Anderson) of the "proximal origins of sarscov2", the paper (which people have been trying to get retracted) that claimed that anyone (inc scientists, virologists, experts) who concluded that the virus came from a lab were just "conspiracy theorists" had said in private to colleagues (now since publicly available) in private emails that quote:

“after discussions earlier today, Eddie, Bob, Mike and myself all find the genome inconsistent with expectations from evolutionary theory”.
