NHS Stockport CCG has won almost half a million pounds of funding to make innovative improvements to health and social care services for its most vulnerable patients
The project is aimed at frail, primarily elderly patients who have multiple long term illnesses and will enable them to access faster and more comprehensive care.
It will mean a reduction in the number of them who are admitted to hospital in an emergency and will lead to a better experience for the patients.
