Health leaders have joined forces with the voluntary sector to launch a new service that will help frail and elderly people in the High Peak stay at home rather than go into hospital.
NHS North Derbyshire, Hardwick and Erewash Clinical Commissioning Groups (CCGs) have contracted NDVA (formerly North Derbyshire Voluntary Action) to provide a Voluntary Sector Single Point of Access (vSPA) service that will go live on Monday.
The service will link local people at most risk of hospital admissions to the extensive range of support services that exist across the voluntary and community sector (VCS).
