The system of holding the NHS to account when things go wrong has broken down and needs radical reform, according to the Public Services Ombudsman for Wales.

Peter Tyndall, who steps down from his post at the end of the month to become the Ombudsman and Information Commissioner in Ireland, proposed that non-executive directors of health boards in Wales should become independent of the executive with their own support staff.

In an interview with WalesOnline, Mr Tyndall said that during his five years as ombudsman there had been a shift from complaints about planning matters ? down because of fewer developments ? towards complaints about the health service.
