I had a chat with my DSN this morning about the way my numbers are behaving. I've noticed a sharp rise in the mornings after the insulin has worn off. She suggested I might split the dose and take 12 units at night and 4 in the morning. Have any of you done this with a long acting insulin? I'm going to give it a go from tonight, but wanted to know if it worked for any of you. My numbers do start to come down on their own after I have my morning Byetta and thoughout the day when I'm most active. Take Wednesday for instance. Woke at half six, BGs 7.1 an hour later they were 9.5. I have trouble eating as soon as I wake because I'm often quite nauseous first thing and even asmall amount of food cancel make me sick.
 
I have never personally split my long-acting Alison, but I know it's a very common practice. Most people are far more insulin-resistant in the mornings (myself included), plus you have the extra boost from the liver going on as you wake to face the day, so it's a good idea to make sure you have active insulin in the mornings to help keep levels steady 🙂
 
I was advised to split my dose 50/50 by the dafne nurse and it worked well.
 
Hi Alison.

I have always been on a split dose of my long acting insulin from day one, mainly because of the large dose. I take 55 units in the morning and 70 units at night.

I have a split dose Alison - many years just doing one, and then about 2 months ago I was told to split my doses as I was running out of long lasting insulin in the evenings when I was exercising.

I haven't had any problems with my levels as a result, I'd even say they are better overall. My biggest thing is the odd occassion I can't remember if I injected it in the morning or not (this is the additional dose).

I'd say it's worth doing and just test, test, test to see how you get on 🙂 Good luck!

That's exactly the question I posed a month ago when you first mentioned Humulin I ....
"It's an Intermediate Insulin whose Peak action is over by 8 hours - it'll keep you in order during the night but what about the day ? What will you do when you eat ? Will you eventually need 2 injections of this per day ?"

Yup - tried it with Lantus at first but things went from bad to worse so asked to change to Levemir - utterly fab. (well in comparison I mean)

Used to have 14u when my feet hit the floor in a morning and a further 4u at approx 9.30pm.
 
Well that's a relief. It wasn't the 1.5 DSN I spoke to so I wanted a bit of reassurance. I'll keep you posted.
