Portsmouth Hospitals NHS Trust and Southern Health NHS Foundation Trust win prestigious national award for their innovative work with diabetes.
The teams were awarded first prize in the Quality in Care Awards for the best network care initiative. The award recognises the work behind the super six model, which has helped to redefine integrated diabetes care across South East Hampshire and its success has meant it has been adopted across Portsmouth as well.
The latest award follows ?hot on the heels' of success in the Care Integration Awards, NHS South East Innovations Award and short listings in the HSJ Awards, which the team is still awaiting the outcome of.
