Pls help me fill in this Diet Questionnaire for my study !!!

Hi all

My name is Yulia, I am a final year Industrial Product Design student. I am currently doing my final year project looking to design a diet product that could help diabetic people to organise their diet.
Therefore, I would need some primary research such as questionnaires or interviews to back up my studies. It will be great if you guys could help me fill them in and send back to me (su-yatang@outlook.com). The information will not be shared to others/ post to public and of course, if you are feeling uncomfortable about some of the questions, you can choose not to answer, just put a N/A next to it.

When I opened it and tried to fill it in i kept getting a message to open some microsoft 365 account in order to access it. I am a tech dinosaur with a new and scary computer so wary of anything like that and therefore i haven't done it.

what other types are there?
there are 5 sub-types or clusters of diabetes. Cluster 1 is type 1 diabetes, clusters 4 and 5 correspond to type 2 diabetes. Clusters 2 and 3 can be thought of as falling between the two extremes, right?
is there a name for cluster 2 and 3?

There is Type 3c where the pancreas has been damaged or removed and LADA which is late onset Type 1 and MODY
