Relationship to Diabetes
Type 2
I am new to this, and needed to find support.I am a type 2 diabetic.I have tried to lose weight for years and have not succeeded.I am now trying the low carb diet which I have done for just over a week,my BM have been raised and ketones in my urine.I am afraid that I am in ketoacidosis. What can I do....

First thing to say if your ketones continue to rise, it is important to get help. Do not ignore this.

What are the circumstances around your diagnosis?
When are you testing your BG. If you are still getting high levels before meals and are on a low carb diet, it would be worth asking for further tests. It is not uncommon for adults to be assumed that it has to be T2, but they are in fact T1.

I have been a Type 2 for 14yrs. I am doing my BG Pre meal and post meal. Did not feel well this am and BG am was 9.9 as soon as I got up. Took my metformin 1000mg with stilagliptin did not eat any thing as did not feel well went up to 11 by lunch time did get a bit frightened. Took metformin 1000mg at 18:00.. hope it will be ok as have to eat something ...Thank you

Hi and welcome.

Do you know what your HbA1c reading was at diagnosis and when that was?

What sort of BG(BM) readings are you getting and when are you testing? Peoples' idea of raised BG can be very different. You really only need to worry about ketones if your BG is in the mid teens or above. If you are just talking about raised BG readings of 8 or 9 then the ketones are not of any real concern and are in fact what you are looking for because they indicate that your body is in ketosis and burning fat.

Thank you so much for that....that makes me feel better, have always hated having Diabetes and now so frightened, with the pandemic.

It may be that your liver is dumping glucose because you didn't have any breakfast.
I see that you have been diagnosed a long time. What was your most recent HbA1c reading? Has it increased over the years?
How low carb have you gone and what sort of BG readings were you getting before you started going low carb?

When you say you didn't feel well this morning.... In what way?

I have not been able to have my HbA1c for 7mths as dr was not doing routine so last years was 59 yes it has increased over the years. Less then 100 carbs a day....BG have been a little on the high side for me pre and post... ie 7-8pre. 11-12 post...only started the diet 2weeks ago.

Felt hot and sweety..this happens when my BG is a little high for me...also felt a little sick could not eat...no appetite all day.

Infections, various medicines, stress and also lack of sleep are things which can significantly raise Blood Glucose.

It's not a good idea to take Metformin on an empty stomach and that could certainly make you feel nauseous. Best to take it with food either mid meal or just after and if you don't feel like eating breakfast then take it at lunchtime, but maybe try some creamy (whole milk) natural Greek yoghurt with a few mixed seeds and/or chopped nuts and a few berries for a light breakfast if you can, as that may help keep your liver from topping up your BG levels.

What sort of ketone reading did you get and when did you test it?

An HbA1c reading of 59 is not overly high and if you have been diagnosed 14 years, it has obviously not significantly increased during that time and the BG readings you are getting now are pretty much consistent with what you would expect with an HbA1c of that level, so I don't think too much has changed to be overly concerned. Obviously you want to reduce it but it is still early days with the low carb diet and 100g carbs a day is not that low. The pre and post meal readings suggest that the amount of carbs you are eating is probably more than your body can tolerate, so it might be worth going a little lower. .... or by pre and post did you mean before starting low carb diet and after?

Can you give us an idea of the sort of things you are eating on the low carb diet? ie a typical day's menu?

Welcome to the forum @Jeanovine

Sorry to hear about your anxiety over ketones.

Do bear in mind that we can only share our own experiences. If you are concerned or worried, or if you continue to feel unwell please contact your GP for a phone appointment or call NHS111, particularly if you begin to feel breathless, get sensations of nausea or abdominal pain.

From what you have said it doesn’t seem to exactly match the things I’ve been asked to look for in terms of ketoacidosis, but I don’t have any medical training!
