A VITAL South Tyneside health service which helps patients discharged from hospital re-settle in their homes is being axed, it has been revealed.

The borough?s Home Care Support Service has been given three-months? notice of closure by health commissioners.

Management met 26 staff this week to inform them of the decision.

And if the staff, who work mainly out of Boker Lane Health Centre in Boldon, cannot be redeployed they will face redundancy.

Today trade union leaders expressed ?shock and outrage? at the move and called on South Tyneside Primary Care Trust to ?reinstate this service immediately?.

The Home Care Support Service was launched in 1994 and provides up to six weeks of NHS support to patients who are discharged from hospital and to patients referred by GPs.

Workers give household support to discharged patients and also provide vital palliative care. The team has been praised as a model in enabling patients to remain in their own home when otherwise they might face a prolonged hospital stay.
