Something seems to be going wrong with consultations in the US, based on this article, if all these people are losing their hypo awareness and don’t even realise that’s a problem. Every time I have an appointment at the hospital or with my surgery nurse, the one thing I always get grilled about is my hypo awareness, and how many I’m having.

Great report, but has anyone else noticed the fact that he hadn’t exactly excelled at the weight loss and presumably the exercise if that photo was reasonably current and post diagnosis. Not mentioned in the report, but 20kg lighter he may not have been medicated at all. And with a proper diet, his A1c would have tumbled down.

But that is the problem in the US, a pill for everything. Proper health advice and supervision doesn’t make profit for the drug companies, though.
 
Then again, not all of the US. Just at the time I was diagnosed in March 2018 the American Collesge of Physicians came out with new guidance: for most T2D's, an HbA1c in the 7% - 8% range is just fine and that physicians should consider reducing or eliminating meds for people at 6.5% or lower.

This was based on the well-known absence of evidence for actual significant clinical harms from an HbA1c of < 8% versus the hypo risks associated with some meds and longer-term risks and costs associated with others.


The guidance included pretty negative assessments of the ADA and the endocrinoligsts' guidelines, citing lack of transparency and inferring (I think) some conflicts.
