Hi everyone,

I am a student at Imperial College London working on a master’s project that aims to empower people with type 2 diabetes in self-managing their condition through the use of online consultations.

As part of my research, I have created a survey where I hope to understand more about your experiences living with and managing type 2 diabetes, as well as your thoughts towards consultations.

I would greatly appreciate if you could take the time to fill it out- It shouldn’t take you longer than 10 minutes. Participation is voluntary and you may withdraw at any time. Your response will remain anonymous however there is an option at the end of the survey to sign up via email to review my proposal for a digital empowerment tool.

Please fill out the survey if:

  • You are over 18
  • You currently live with type 2 diabetes
