Diabetes patients have become the second group to have their care moved closer to home.
North Kirklees patients no longer need to go to Dewsbury hospital after health chiefs re-located services to a range of health centres, GP surgeries and in their own homes.
An estimated 10,000 diabetes patients are set to benefit from the service re-design.
Last month kidney patients became the first within Mid Yorkshire NHS Trust to have their outpatient services moved out of hospital.
North Kirklees Clinical Commissioning Group (NKCCG), who are now controlling the service, has appointed two new community diabetes nurses – Kathryn Jolly and Jo Bissel.
