Investigation launched into the handling of complaints into 250 deaths which may have been avoidable.

The health service watchdog is to investigate allegations about the needless deaths of more than 250 patients amid concerns that many may have been the victims of “appalling” failings.

Dame Julie Mellor, the NHS Ombudsman, told The Telegraph that she has ordered the systemic investigation because of evidence that too many deaths are never properly examined.

The head of the watchdog - which investigates complaints when health services are accused of failing to handle them fairly - said she was concerned about the “incredibly variable” quality of efforts by the NHS to investigate avoidable deaths.

The probe will examine cases referred to the Ombudsman by families who believe the deaths of their loved ones were avoidable, but were never properly investigated.
